Qualifications
Nurses - One (1) year of recent acute care experience is necessary to provide the high degree of skills demanded by clients. The nurses referred will have met the required criteria for placement.
Phlebotomists - One (1) year experience
Providers/Companions/Sitters - One (1) year of experience.
Skills Assessment
This is to determine your nursing strengths appropriate to your job classification.
Licenses
You must possess a current License appropriate to your job classification. This license MUST be carried on your person at all times while working at the client hospital.
Evidence of Good Health
- You must have a completed physical exam on date of hire. The following must also be on record in order for you to work:
- You must have a current chest X-Ray or TB skin test
- Rubella Titer
- Rubeola Titer (if born in 1957 or prior)
- Varicella
- Hepatitis (optional)
- If your TB skin test is negative you must have one done yearly. If your TB skin test is positive a chest X-Ray must be done every 3 to 5 years or, yearly if there are signs of TB infection.
CPR
American Heart Association Basic Cardiac Life Support (BCLS) is the only CPR certification accepted. Each nurse shall carry a copy of his / her BCLS certification and shall present said certification to the hospital representative upon request. It is required that all nurses for Critical Care Specialties (PALS, NRP) in addition to the above BCLS criteria also must carry the ACLS certification and present it to the hospital representative upon request.
Background Check
As a condition of employment On-Call Healthcare will run a criminal and educational background check.
Drug Screen As a condition of employment On-Call Healthcare, Inc. requires a 10 panel drug test with a negative result.

1. Application, please fill out completely & accurately.
2. W-4 form, please complete for payroll.
3. 1-9 form, please complete top portion only.
4. Personal Medical History Questionnaire, complete & sign.
5. Hepatitis form: include dates of vaccine series, titers, booster's or other pertinent information & sign.
Please include these documents when faxing back the above items:
1. Nursing license or Certificate
2. CPR Card
3. ACLS, PALS, NRP, EMT Card
4. Drivers License
5. Specialty Certificates, (i.e. CCRN, NICU, ER, OB etc.)
6. Physical Exam & TB test or CXR Documentation. Physical/TB must have been
done within the last year. You may request a Health Certificate Form from the office if you need exam to been done. Physical only needs to state you have not physical limitations and can work.
7. Immunizations / Titer Records including Hepatitis Series & MMR Vaccine and the results.
8. Drug Screen is required along with your pre-employment physical unless you can provide us with a recent copy in the past 6 months done by your previous employer.
9. Name, address & phone # of (2) Work related references that will comment on your MEDICAL EXPERIENCE.
